As you may recall, in July 2000, we distributed the Physician Education Video and Documentation Poster (PEPP Administrative Memorandum #2000-10). Since then, we have received considerable feedback from hospitals regarding the need for improved physician medical record documentation to further support ongoing efforts to reduce inpatient payment errors.
As part of our continued collaborative work with hospitals, we are sending the enclosed "Docu-mentation Prompter" stickers for your use. (Please note that while these stickers outline important areas to be addressed by physicians in their progress notes, they do not necessarily include all documentation requirements). Please consider placing them on, or in, the medical record as documentation reminders for your physicians. We have also enclosed a diskette of the sticker so that you can reproduce it in other formats, such as prompter pages to insert in the medical record or as a poster to place strategically in the hospital.
