IPRO, the Medicare Quality Improvement Organization for New York State, will be sponsoring a statewide Home Health Conference Call Series entitled "Clinical Strategies to Improve Patient Outcomes".

This conference call series will address strategies and interventions targeting the complex issues involved in reducing avoidable acute care hospitalizations, including communication with physicians and collaboration across health care providers to support improvement in patient-centered care transitions. The presentations will also include information on the importance of data management and utilization of rapid cycle quality improvement. The purpose of this conference call series is shared learning to support your agency's Outcome-Based Quality Improvement (OBQI) efforts.

Enclosed please find a schedule of the conference call presentations that includes the presentation topic area, date, time and call-in information. There is no registration fee for participation and you are welcome to join any of the conference calls that may be of interest to you. We strongly encourage you to take advantage of this educational opportunity and also to include the administrative and senior management representatives of your organization. Agencies who are members of the Clinical Performance Identified Participant Group are requested to register for participation on each call and to remain on the line from 10:00 AM - 10:30 AM for a monthly meeting with the IPRO Home Health Project Team.

Please indicate on the attached Registration Form all of the calls that are of interest to you as we want to ensure that there will be adequate phone lines available. If you are not sure at this time which of the calls you would like to join, for your convenience, we will send reminder notices in advance of each call so that you will have another opportunity to register. The reference materials for the individual presentations will be forwarded to registrants via email in advance of each call.
