In support of performance improvement of the Acute Care Hospitalization & Improvement in Oral Medication Management quality measures, IPRO will be conducting two regional educational conferences for the home health provider community. These conferences will focus on optimizing the role of the Home Health Aide as a valued member of the care management team to improve patient outcomes.
Representatives from the Visiting Nurse Service of New York will discuss the Learning Collaborative Model utilized by their organization to improve communication and collaboration with the Home Health Aide and the benefits achieved from this partnership in care management targeting improved patient outcomes. The session will also include a presentation from a representative of the Paraprofessional Healthcare Institute that will address the current working environment of the Home Health Aide & issues involving recruitment & retention. This will be followed by a sharing session to discuss strategies agencies can initiate to facilitate involvement of the Home Health Aide to achieve the care management goals established within the patient's plan of care.
We strongly encourage administrative & supervisory representatives from Home Health agencies serving the long term care patient population to attend this conference.
