IPRO, the Medicare Quality Improvement Organization for New York State, will be sponsoring a statewide Home Health Shared Learning Teleconference Presentation Series entitled "Impact of Home Health Organizational Culture Change on Improving Patient Outcomes".

This shared learning teleconference series will provide the opportunity for individual New York State home health providers, who have established an organizational culture that supports improved care management, to share their success stories regarding comprehensive care management and improved patient outcomes. These agencies will discuss their quality improvement initiatives and the interventions that have contributed to their ability to maintain low acute care hospitalization rates and improvement in patient outcomes through utilization of organizational culture change concepts. The purpose of this teleconference series is shared learning among your peers to support your agency's Outcome-Based Quality Improvement efforts.

Enclosed please find a schedule of the conference call presentations that includes the presenters, date, time and call-in information. There is no registration fee for participation and you are welcome to join any of the six conference calls that may be of interest to you. We strongly encourage you to take advantage of this educational opportunity and also to include the administrative and senior management representatives of your organization.

Please indicate on the attached Registration Form all of the calls that are of interest to you as we want to ensure that there will be adequate phone lines available. If you are not sure at this time which of the calls you would like to join, for your convenience, we will send reminder notices in advance of each call so that you will have another opportunity to register. The reference materials for the individual presentations will be forwarded to registrants via email in advance of each call.
