In support of the Centers for Medicare & Medicaid Services (CMS) 8th Scope of Work initiative to reduce avoidable acute care hospitalizations (ACH), IPRO is pleased to invite you to attend a workshop meeting to assist your agency in the design, development and implementation of a Plan of Action addressing acute care hospitalization.

These workshops will provide an opportunity for your agency to receive one-on-one assistance in finalizing a Plan of Action targeting reduction in avoidable acute care hospitalization. The workshop will build on the information provided on the October 20th and 27th IPRO conference call sessions addressing utilization of the Acute Care Hospitalization Improvement Matrix for selection of evidenced-based strategies and interventions for this quality measure. In order to attend the workshop you will need to have completed a Process of Care Investigation to identify the specific problem areas that are driving your agency's ACH rate. The agenda will include information on the ACH Improvement Matrix, selection of strategies and intervention actions to incorporate into Plan of Action development and review of tools to support ongoing monitoring of the interventions you choose to focus on for this quality measure.

Meeting Dates/ Locations - All workshop meetings will be held from 9:00 am to 4:30 pm.

Due to space limitations we will not be able to accommodate more than two representatives from each agency. There is a fee of $25.00 per person to attend the workshop to help defray the cost of food and beverages. We accept credit cards and checks as forms of payment.
